Enrollment in Rocky Mountain PACE is available to individuals who meet specific eligibility criteria. Applicants must be 55 years of age or older, reside within the designated Rocky Mountain PACE service area, and be assessed by the State of Colorado as requiring the level of care typically provided in a nursing facility. Additionally, individuals must be able to live safely in the community at the time of enrollment, as determined by guidelines outlined in the Enrollment Agreement.
